As to what could have caused the problem, it's probably some kind of corruption in your system or settings files. The first thing I always recommend is to make sure your hard drive is in good shape by [repairing the hard drive with Disk Utility http://support.apple.com/kb/TS1417]. If there aren't any problems, you've got to move on to other possibilities, but if there are problems, fixing them may help. Any time you have problems that Disk Utility can't repair, you need to 1) ensure the immediate safety of your data, and 2) try repairing with something like [DiskWarrior http://www.alsoft.com/DiskWarrior/index.html] or [TechTool Pro http://www.micromat.com>. (As an alternative to #2, you could just erase the hard drive and reinstall everything from scratch... I often prefer this method in such a case, as damage that cannot be repaired is severe enough that I usually assume that some corruption has resulted somewhere because of that damage, and I pre-emptively strike to remove that corruption. That's probably overkill sometimes, but ensures I've got a system I can rely on.)
